Applications are available to be picked up and returned on Tuesdays and Thursdays ONLY from 9:00 A.M. to 12:00 P.M. at 211 N. Ridgewood Avenue, Suite 200, Daytona Beach, FL. You may also fill and print an application online, click here.

The waiting list is ordered by date and time ONLY. The waiting time for assistance is an estimated six (6) months to one (1) year. You will be notified of your eligibility within 30 to 60 days.
